What Your Medical Plan Covers

Pick your plan once and the whole page adjusts to show only your costs. Your plan year is set by your employer — ask your HR team for your dates.

The short version. This plan uses the Cigna nationwide PPO network. In network, preventive care and virtual care are $0 on every plan. Chiropractic and acupuncture are a $20 copay on seven of the eight plans. Most facility and imaging services are 20% coinsurance after your deductible. Prescriptions run through DisclosedRx, starting at a $15 Tier 1 retail copay. Going out of network costs substantially more — usually 40–50% after a doubled deductible.

Rehabilitation Services

20 visits per year. Some services may also need pre-certification.

Habilitation Services

20 visits per year. Some services may also need pre-certification.

Skilled Nursing Care

60 days per year.

Children's Eye Exam

One exam every 24 months, except where the Affordable Care Act requires it more often.

Pre-Certification

Some procedures need approval before you have them. Your provider usually starts it — confirm it went through before your appointment, not after.
